The U.S. Fish and Wildlife Service invites offers on a proposed contract for Road Rehabilitation at the Canaan Valley National Wildlife Refuge. The Service will utilize the two-step bidding process for this procurement in accordance with the Federal Acquisition Regulation. A firm-fized price contract is anticipated. (3) Technical Skills - Qualifications your company or subcontractor have that relate to the proposed work and how long these employees have been with the company (carries a total possible weight of 15%). (4) Capability to Perform - Financial, labor, equipment and material resources your company and subcontractors have (carries a total possible weight of 15%). (5) Safety Record - In the last five years, how many lost workdays and how many Occupational Safety & Health Administration (OSHA) or West Virginia Department of Labor safety violations have your company or subcontractors had (carries a total possible weight of 10%). (6) Labor Standards - How many violations of West Virginia Department of Labor or other state or Federal labor pay rate regulations have your company or subcontractors had (Carries a total possible weight of 10%). (7) Past Performance - Provide at least five references including name, address, and telephone number of owners of similar projects that have been completed in the last five years by your company or your company and the same subcontractors proposed to be used on this project (carries a total possible weight of 20%).
