The weed control services to be provided at Four Corners Regional Health Center, Red Mesa, AZ 86514.

Note: Contractor to indicate services will be a one-time service.

Services to provide in accordance with the Statement of Work and Service Contract Act Wage Determination NO: 2015-5446, Date of Revision: 12/23/2019.

Service to include the following: Estimated 93 housing units, driveway, streets, sidewalks and 12 acres of Health Facility site. Weed control in the areas indicated on the attached Statement of Work as well as the requirements.
