THIS IS A SOURCES SOUGHT NOTICE to identify interested firms capable of performing rat eradication services on a remote island (Wake Island Atoll) in the Pacific Ocean for the U.S. Air Force.

Responses shall include the following:

(1) List of experience(s) with similar work (type and scope) to include contract numbers, project titles, dollar amounts, and points of contact with phone numbers.
(2) Completed questionnaire.

Firms interested shall provide the above documentation via e-mail to [email protected] no later than 3:00PM Hawaii Standard Time, on Friday, 21 Aug 09. All information submitted shall be in sufficient detail and will be used to assist with a determination of the the most appropriate acquisition strategy. Responses shall be limited to no more than 10 pages total. Failure to submit all information requested will result in a firm being considered "not interested" in this requirement.

Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.

QUESTIONNAIRE

1. Do you have experience in the use of 1) helicopters for broadcasting rodenticide, 2) hand broadcast of rodenticide, and 3) rodenticide bait stations?

2. Have you successfully conducted a rodent eradication on a large island?

3. Do you have experience working on tropical islands with large populations of land crabs that can interfere with a rodent eradication through scavenging bait?

4. Do you have experience purchasing rodenticide under the EPA and USDA's conservation label? (Rodenticide purchased under this label is restricted to "Conservation Agencies")

5. Do you have experience working with the US Environmental Protection Agency (EPA), United States Fish and Wildlife Service (FWS), and National Oceanic and Atmospheric Association (NOAA), for proper implementation of a large scale rodenticide application?
