The Indian Health Service (IHS) is conducting a SOURCES SOUGHT/REQUEST FOR INFORMATION as outlined in FAR 15.201(c)(3) to gain knowledge of interest, capabilities, and qualifications of firms especially small businesses including: Native American/Indian-Owned Businesses (Indian Economic Enterprise (IEE)), 8(a) Certified Small Businesses, Historically Underutilized Business Zones (HUB-Zone) Small Businesses, Women Owned Small Businesses, Service Disabled Veteran-Owned Small Businesses (SDVOSB), Small Disadvantaged Businesses (SDB), and Veteran Owned Small Businesses.
Interested firms are requested to reply to this announcement with a submittal as described below. Please note that a decision not to submit a response to this request for information will not preclude a vendor from participating in any future solicitation.

Market Research is being performed pursuant to Federal Acquisition Regulation (FAR) Part 10 to identify businesses capable of performing the functions described herein.
SUMMARY OF SCOPE
The contractor shall provide all labor, materials, supplies, and services to design and build up to four projects for the Wagner Indian Health Service Clinic:
Flooring: Provide and install flooring similar to Alto Wood LRV Bavarian Oak, Model WSA2026. The contractor shall follow installation guidelines of the flooring manufacturer. Install vinyl wall base as well. Install Tarkett Johnsonite 6” vinyl wall base. Submit on color. The floor covering and base shall be recommended for use in a health care facility. Coordinate with facility on any phasing.
Exterior Improvements:
Painting: This project will paint the brick exterior on the west wing to match the brick of the east wing, paint the window and door frames a dark bronze color to match new doors and windows. Paint facia.
Tuckpointing: Tuckpoint brick of building that is needed. Not all brick is needing tuckpointing, but same small areas will, roughly 1200 square feet.
Windows: Replace the needed existing window panes, with fogging and no longer sealed, with energy efficient glazed windows.
Propane Fuel Tanks: Install: A new propane system to include tanks, vaporizer, lines, and leak detection shall be installed, following project plans. The boilers shall be converted to propane usage. The old line shall properly be retired so that no leaks will occur.
Plumbing: Repair the two 4” solder copper leaks on the water manifold that were found while
rebuilding the manifold. Plan with the facility for a water shut-down, and repair when most convenient for the patient care.
In accordance with FAR 36.204, Disclosure of the Magnitude of Construction Projects, the anticipated project magnitude is between $250,000.00 and $500,000.00
The type of work to be performed will be categorized under PSC code Y1DA, Construction of Hospitals and Infirmaries and NAICS code 236220, Commercial and Institutional Building Construction with a size standard of $45 million.

For Native American/Indian Owned Businesses only, the contractor shall submit the controlling enterprise’s Department of the Interior, Bureau of Indian Affairs Certificate of Degree of Indian Blood documentation. Please note that there shall be no personally identifiable information (PII), such as social security numbers, included in the documentation submitted. Any PII shall be redacted prior to submission.
