A quotation is requested to provide Verbenone Pouches and MCH Bubble Caps for Region One and Region Four. The awarded purchase order or purchase orders will be firm fixed price purchase orders, with delivery required on April 15, 2016 and May 27, 2016.

The FAR provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. In addition, offerors will include with their offer a completed copy of the FAR provision 52.212-3, Offer Representations and Certifications - Commercial Items. The FAR clauses 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition.
