Due to the value of this contract, DOE requires Business Representations and Certifications be completed for all procurements exceeding $10,000.  Please reference Solicitation Number VS386157 on the form and all correspondence.  Please complete and return all twelve (12) pages of the document.

1.          Published Price list: showing the items on your quote for price justification or a prior customers invoice for the same/similar items (personal info whited out). 

1.          Insurance:  **IF APPLICABLE** Upon award, in accordance with Article 6 of the attached “BSA Supplemental Terms and Conditions for Work by Contractors On Site at Brookhaven National Laboratory”.  Prior to coming on site to perform work, the certificate must remain in effect for the duration of the subject contract and must be filed with BNL Procurement & Property Management Division.

1.          System For Award Management (SAM):

In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Dun & Bradstreet number (Duns#). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization.  Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. 1.          Basis of Award:

BSA will make award to the responsive and responsible offeror who submits the lowest total price and who meets all of BSA’s technical requirements, as stated herein.
