Provide labor, materials, and transportation for courier services for weekly delivery of dry ice (approximately 5 pounds to 7 pounds) to the St. George Community Based Outpatient Clinic on Fridays, and to the VA Pocatello, Idaho Community Based Outpatient Clinic on Mondays. The dry ice is required for transportation of labs to the George E. Wahlen Veterans Affairs Medical Center in Salt Lake City, Utah. The contract will run from April 1, 2006 through March 31, 2007 with four one year options to renew the term of the contract. The contract will be an indefinite quantity commercial contract.
