The contractor shall provide all parts(including toggles, etc.), labor, on-site supervision, uniforms and identification, equipment, tools, transportation, supplies, and materials necessary to perform all tasks related to the transportation of furniture/equipment, installation/de-installation of government owned new and used free standing, wall mounted and modular/systems furniture, reconfiguration, reupholstering and cleaning furniture in a hospital/clinical setting. The contractor shall be located within a 150 mile radius of the VA Ann Arbor Healthcare System. Contract is for the period of the Date of Award through September 30, 2007 plus four (4) one (1) year option(s) for the VA Ann Arbor Health System (VAAAHS), 2215 Fuller Road, Ann Arbor, MI 48105 and ancillary facilities located at: VA Toledo Outpatient Clinic, 3333 Glendale Avenue, Toledo, OH. 43699; VA Flint CBOC, G-3267 Beecher Rd., Flint, MI. 48532; VA Jackson CBOC Townsend Medical Center, 400 Hinckley Blvd., Suite A, Jackson, MI. 49203 and any new clinics which may be developed in the VAAAHS supporting area as well as facilities at; National Center for Patient Safety, 24 Frank Lloyd Wright Dr. Lobby M, PO Box 486, Ann Arbor, MI. 48106, and VISN 11 Healthcare Network, 24 Frank Lloyd Wright Dr. P.O.
