36C25624Q0701 RFQ SOLICITATION

The Mental Health Care Line Department at the Michael E. DeBakey VA Medical Center, located at 2002 Holcombe Blvd., Houston, TX 77030 has a requirement for Yoga Therapy.

The contractor shall provide yoga therapy to patient populations of Veterans at the Michael E. DeBakey VA Medical Center (MEDVAMC). Contractor’s personnel shall perform services via video teleconference (VTC). Contractor shall provide all access to the necessary resources to accomplish two-way, interactive, virtual platforms to offer online classes and individual sessions of live yoga therapy.

The solicitation will be issued as a Small Business (SB) set-aside under NAICS code 611699, with a small business size standard of $16.5 Million. All interested parties must be listed in the System for Award Management (SAM) at https://www.sam.gov, at the date and time of their quote submission, and date of award.  Award will NOT be delayed for registration.  

SPECIAL INSTRUCTIONS

1. All quotes must be submitted directly to the Contracting Officer identified and listed in block 7a of the Standard Form (SF) 1449, by email to the following address: [email protected]. Quotes must be received no later than the date/time indicated in block 8 of the SF 1449. Any proposal received after the date and time referenced in block 8 may be considered late and may not be accepted.

2. All emails must reference “RFQ 36C25624Q0701, “Yoga Therapy” in the subject line of the email. Failure to do so may result in mishandling or inadvertent deletion of emails.

3. QUESTIONS: All questions regarding this solicitation must be submitted to the Contracting Officer by email, no later than 12:00 PM, Thursday, May 23, 2024.

4. SUBMISSION REQUIREMENTS: All quotes must include the following, as a minimum. Failure to include any of the following with a quote may result in that quote being determined non-responsive. Any quote determined non-responsive will NOT be evaluated for award.

a. Contractor shall submit proof of updated certifications with quote as outlined in Attachment 1 - Statement of Work, Section 3.2.
