SCOPE OF WORK ID #: LRJC2014-017 OPTICAL SERVICES Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Little Rock Job Corps Center - a 237 student residential training facility located at 6900 Scott Hamilton Road, Little Rock, AR 72209, is currently soliciting written bids for optical services. All quotes must be submitted in writing to the Purchasing Department by Friday, February 28, 2014 by 3:00 p.m. Quotes received after the deadline and/or without all requested documentation will not be considered. Bid MUST be good for 90 days, after submission. No partial bids will be accepted. Vendor is to return written bid on vendor letterhead and LRJC2014-017 at the top of each page, vendor paperwork, and all licenses for doing business in the State of Arkansas, all employee licenses required for performing work in the state of Arkansas, Certificate of Liability insurance and list of references if no past experience with Center. CONTRACT PERIOD: This contract shall commence on April 1, 2014 and continue through March 31, 2015. Overall low bid does not constitute bid award. Little Rock Job Corps Center is a federally funded facility and entitled to GSA pricing. Payment terms are Net 30 days from date of invoice. Vendor is required to show number of hours billed and hourly rate on invoice, list of materials used to complete service and provide certified payroll records with invoice. All wages are to be paid per Davis Bacon, see attached, and subject to the Service Contract Act. SCOPE OF WORK ID #: LRJC2014-017 OPTICAL SERVICES Vendor is to provide optical services every second Tuesday of the month between the hours of 9:00 a.m. and 11:00 a.m. Vendor shall hold all licenses necessary to do business in the state of Arkansas and provide copies of all licenses with bid package. Vendor will see a maximum of 10 students per visit and perform a comprehensive eye exam and fitting for glasses if needed. Vendor shall provide laboratory services to fill prescriptions for glasses, replace broken lenses, as well as adjustment and repair of frames as required. All eyeglasses must meet American National Standards Institute (ANSI) standards. Vendor will provide Wellness Manager with a copy of the full examination to place in the student's file. Vendor will abide by all HIPPA policies. Vendor is to provide a billing invoice that includes BPA number and shows individual lines for each item ordered. Little Rock Job Corps Center does not accept backorders all orders will be considered complete upon receipt. Vendor must make Purchasing Agent aware of all shortages, via email ([email protected]) prior to shipping order. The vendor shall obtain and maintain insurance as follows if vendor's employee(s) or agent(s) will be on Center at time: • Workers Compensation • Comprehensive General Liability with Little Rock Job Corps Center listed as additional insured. ($1,000,000 per occurrence). • Comprehensive Automobile Liability ($1,000,000/combined single limit) • Fire Legal Liability ($500,000 per occurrence) • The proposed shall cover any and all expenses not covered by said insurance including all deductibles.
