The purpose of this Amendment is to change the solicitation posting and closing dates. The new posting date of this solicitation will be December 3, 2014 and will close on December 23, 2014.  Please read the notice below:


The Federal Bureau of Prisons, Transitional Drug Abuse Treatment Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Cleveland, Ohio. The location where services are to be provided should be within a 10 miles radius of Cleveland City Hall, 601 Lakeside E., Cleveland, Ohio 44114 and accessible to local public transportation. Performance periods under this contract will be for a one-year base period to begin on March 01, 2015 with four (4) one-year option renewal periods with service ending (if all options are renewed) on February 28, 2020. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor.

Base Year: Estimated number of inmates = 100

2011- Intake Assessments (one per inmate) for 100 inmates = 100 assessments
2010 - Individual Counseling Sessions (30 minutes = 1 unit) 24 units (per inmate) x 100 = 2,400 units
2020 - Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 100 = 7,200 units
5011 - Mental Health Intake and Assessment (one per inmate) for 15 inmates = 15 units
5030 - Psychiatric Evaluation and Report = 1 report per inmate x 15 inmates = 15 reports
6010 - Individual Mental Health Counseling (30 minutes =1 unit) 24 units per inmate for 15 inmates = 360 units
6020 - Group Mental Health Counseling (30 minutes =1 unit) 12 units per inmate for 15 inmates = 180 units
6051 - Medication Monitoring (6 units per inmate) for 15 inmates = 90 monitoring units

The services shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) most recent Statement of Work dated April 2011- Masters Required, Revision 01, 2013, PSB. The request for quote will be available on or after December 3, 2014 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted.
