THIS IS A SOURCES SOUGHT NOTICE. The Defense Logistics Agency (DLA) Troop Support Pacific is seeking sources for medical reagents, calibrators, media plates, and consumables for Sysmex XT-2000i, CA-1500, CA-560, CA-1800i and Vitek II analyzers.

The Government is contemplating a Fixed-Price Purchase Order with a base period of three (3) months with two (2) one-year option periods. The delivery location will be U. S. Naval Hospital Okinawa. The applicable NAICS code is 325412 - Pharmaceutical Preparation Manufacturing and the small business size standard is 750 employees.

Interested parties shall provide the following information to the Government: (1) Name of Company - include company address, a contact person's name, telephone number, and email address; (2) Company size (please specify as either large, small business, etc.); (3) Describe in detail the firm's capability to provide the items listed below, along with the item lead times. Respondents should include applicable Duns and Bradstreet Number (DUNS), Tax Identification Number (TIN) and System for Award Management (SAM) Commercial Government Entity (CAGE) code number.
