Supportive housing models address the need for those individuals who are not yet ready for independent living and/or who would be served better in a sub-acute setting as opposed to inpatient levels of care or unnecessary waits in hospital emergency rooms. Supportive housing models help to provide support and navigation through a complex system of health needs such as permanent housing, benefits attainment and management and linkage and follow up with primary care. These housing options may vary on a continuum from fairly or somewhat restrictive to increasingly independent models.
