This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. Solicitation Number FA4417-10-S-0001 is hereby issued as a Request for Proposal. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38, effective 10 December 2009. This acquisition is 100% set-aside for Small Business concerns. The North American Industry Classification System (NAICS) code for this project is 713940. The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide the following items: Contract Line Item (CLIN) 0001, Mom/Dad/Caregiver and Baby work-out (stroller work-out) program for the Hurlburt Field Youth Center as defined in the Statement of Work. This requirement will result in a concessionaire agreement between the awardee and the 1 SOFSS. This agreement is for a period of one base and four (4) one-year option periods.

Delivery Terms are FOB Destination for delivery to 1SOFSS/FSFY Building 90304 Hurlburt Fld 32544-5267.

The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors - Commercial Items (June 2008). It is amended to read: Offeror shall submit signed and dated offer to 1 SOCONS/LGCA, Attn: Ms. Gwen Lewis, 350 Tully Street, Building 90339, Hurlburt Field FL 32544-5810, by 24 March 2010 at or before 1:00 p.m. CST. Submit signed and dated quotations Schedule of Compensation marked with solicitation number FA4417-10-S-0001. The government reserves the right to evaluate proposals and award a contract without discussions with offerors. FAR 52.252-2, Clauses Incorporated by Reference (Feb 1998); FAR 52.252-6, Authorized Deviations in Clauses (Apr 1984); NAF GENERAL PROVISIONS (Attachment 1- 9.d.), Hurlburt Local Clauses HF 019, HF042, and HF043 (Attachment 1 - 9.e.).

Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Proposal, with an addendum to paragraph (a) as follows: The following factors shall be used to evaluate offers: (i) technical capability of the item(s) offered to meet the Government requirement as stated in the attached Statement of work. Technical Capability of the offeror is defined as the offeror possessing certification in Cardiopulmonary Resuscitation (CPR) and basic first aid listed in attachment and the offeror's certification in a nationally recognized stroller work out program for post pardum mothers (ii) Price. The Government will enter into an agreement with the technicaly capable and responsible offeror whose offer with the lowest price. Technical capability is significantly more important than price. The offeror shall submit with their offer; completed offer schedule, certification documentation for instructors forecasted to work on this requirement.
FAR 52.212-3, Offeror representations and Certifications (Aug 2009) - Commercial Items, and the offeror must include a completed copy of this provision with their proposal or indicate (with DUNS Number) that information is available on the federal government website ORCA (https://orca.bpn.gov/).
