The Albuquerque Area Indian Health Service has a requirement for a Speech Language Pathologist. The services are required at the Acoma-Canoncito-Laguna Service Unit (ACLSU), located in San Fidel, New Mexico.
The solicitation is anticipated to result in an award of a Blanket Purchase Agreement (BPA). The Request for Quote (RFQ) No. 11-242-SOL-00006 will be posted at www.fbo.gov on November 24, 2011.
Prospective Offerors are invited to submit quotes in reference to RFQ No. 11-242-SOL-00006. The solicitation document comprise of all information including the estimated requirements, scope of work, and applicable attachments. RFQ No. 11-242-SOL-00006 and all accompanying correspondence can be downloaded by clicking on the file links besides the Solicitation posting.
If Offerors elect to submit quotes electronically; submission of original hardcopies either by US Postal Service or hand delivery is required. Hardcopies of vendor quotes to be mailed to the following address: Albuquerque Area Indian Health Service, Division of Contract and Grants Management, 5300 Homestead Road NE, Albuquerque, New Mexico 87110.
The RFQ due date is close of business 4:30 pm MS on December 14, 2010. Late quotes may be considered if they were mailed via USPS and post marked prior to the time and date of the due date, and/or it is the only Offer received.
All quotes received will be evaluated and considered for award. Award of a BPA will be made to the Offeror whose quote is considered fair and reasonable.
The Contracting Officer is the only individual who has the authority to legally commit the Government for the expenditure of public funds in connection with this proposed acquisition.
