Issuing Procurement Unit State of Utah Division of Purchasing Conducting Procurement Unit State of Utah Division of Wildlife Resources REQUEST FOR QUOTES South Canyon Sevier River - Aerial Herbicide Application - Tebuthiuron (Spike 20P) Treatment SOLICITATION #JH20057 This Request for Quotes "RFQ" is issued in accordance with State of Utah Procurement Code and the Utah Administrative Code. If any provision of this RFQ conflicts with the Utah Procurement Code or the Utah Administrative Code, then Utah Procurement Code and then Utah Administrative Code will take precedence. Purpose of this Solicitation: The purpose of this RFQ is to enter into a purchase order with the responsive and responsible vendor that submits the lowest cost to provide: provide for improved wildlife habitat by aerial application of the herbicide Spike 20P (Tebuthiuron or equivalent herbicide with tebuthiuron as the active ingredient @ 20%) . Closing date and time The closing date and time for this sourcing event is October 9, 2019 at 2 pm Mountain Time. SITE SPECIFICS: Location and Description: Work under this contract is located on BLM and State of Utah administered lands approximately 4 miles north of Hatch, UT in Garfield County. Elevations range from 6,800 to 7,200 feet. The work location is shown on the attached Project and Reference Maps. The treatment boundaries have not been flagged, and it is recommended that the contractor provide a GPS or use the PDF maps attached and the Avenza app. The work location is shown on the site maps. Acreage: 242 Acres +/- dependent upon bids and budget constraints(Spike 20P – Tebuthiuron) as identified on the attached site map. Access to the site: The project area is accessible via gravel/dirt roads that begin connected to UT Hwy89. Access to the treatment areas are adjacent to the dirt roads. See attached location map for more details. SPECIFIC TASKS: Work Limits: Only areas shown on the Project Map will be treated. The treatment boundaries have been digitally delineated. It will be required that the contractor provide a GPS such as AgNav in the aircraft to ensure proper aerial application of the herbicide. The work location is shown on the site maps. A GIS/GPS shapefile will be provided prior to work beginning. Contractor shall furnish all labor, equipment, supplies, and materials to aerial apply herbicide on 242 acres. Contractor will need to provide enough labor and equipment to complete the work in the specified performance window. It is the contractor’s responsibility to ensure that the proper herbicide application rate is followed as described in this contract and on the herbicide label. Government staff has identified unit boundaries for the work executed under the contract. The contractor will be required to complete purchase orders with a qualified chemical distributors to provide: tebuthiuron (Spike 20P or equivalent chemical with tebuthiuron as the active ingredient @ 20%) and the aerial application at the rate of 2.5lbs/Acre. Note: If equivalent herbicide is utilized it must be approved by the government prior to purchase and application. The contractor will be required to supply appropriate equipment that can be calibrated to deliver herbicide at specified ratios. Tebuthiuron (Spike 20P) – 2.5 lbs/acre Acreage: 242 Acres +/- dependent upon bids and budget constraints. Note: The contractor is expected to apply herbicide early in the day to avoid windy conditions and minimize herbicide drift. The government reserves the right to postpone operation if wind speed exceeds 10 miles per hour. CONTRACTOR FURNISHED EQUIPMENT: Equipment and Supply Requirements: A fixed wing plane able to meet all of the requirements of the contract will be required for this project. All equipment and/or materials incorporated into the work covered by this contract shall be of the most suitable grade for the purpose intended, unless otherwise specifically provided in this contract. All work under this contract shall be performed in a skillful and competent manner. The State may require, in writing, that the contractor remove from the work any employee deemed by the State to be incompetent, careless, or otherwise objectionable. GPS Technology: The aircraft must be equipped with a global positioning system that is used to establish flight lines for 100% coverage. DWR will provide to the Contractor an electronic file containing coordinate positions of treatment areas in an ARC/INFO GIS format. Contractor shall provide a Trimble, ARC/INFO or ARCVIEW electronic file in UTM projection North America Datum 1983 of treatment coverage upon completion of each treated area or as required by the Contracting Officer. A government representative shall monitor the Contractors GPS to assure Contractors compliance with contract specifications. Herbicide: The contractor will be required to complete purchase orders with a qualified chemical distributors to provide: Tebuthiuron (Spike 20P or equivalent chemical with Tebuthiuron as the active ingredient @ 20%). The application rate of Spike 20P will be at the rate of 2.5lbs/Acre. Herbicide Application: The Contractor shall certify to DWR they have the capability to meet the requirements of the bid prior to performing any aerial work. Length of the Contract Project Performance Time: Ten (10) calendar days from official Notice-to-Proceed (NTP) Extended performance window: (either calendar days from NTP or beginning & end dates) Radio Communication: The Contractor shall provide a communication system so that contractor personnel engaged in the project at different locations can communicate at all times with each other and so that Government Project Inspectors may communicate with the contractor at any time to discuss performance matters. This solicitation number must be referred to on all quotes, correspondence, and documentation submitted to the State relating to this RFQ. Evaluation of Quotes: Each quote received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. This RFQ shall be evaluated by the objective criteria described in this RFQ. Quotes submitted must comply with the specification identified in this RFQ. Any quote that does not comply with the prerequisite and questions sections will be deemed non-responsive and will be rejected. Multiple or alternate quotes will not be accepted. If a vendor submits multiple or alternate quotes, the State will only accept the bidder's primary bid and will not accept any other bids constituting multiple or alternate bids. Any exceptions to the content of this RFQ, including the specifications, must be protested in writing to the Division of Purchasing prior to the closing date. Vendors must review each section of this RFQ carefully before submitting a quote. The Division of Purchasing may solicit other quotes via email, phone, or other methods as permitted by the Utah Procurement Code or applicable Administrative Rules. Vendors are prohibited from communications regarding this RFQ with the conducting procurement unit staff, evaluation committee members, or other associated individuals EXCEPT the State of Utah Division of Purchasing procurement officer overseeing this RFQ. Wherever in this RFQ an item is defined by using a trade name, brand name, or a manufacturer and/or model number, it is intended that the words, “or equivalent” apply; and invites the submission of equivalent products by the Bidders.
