The Federal Aviation Administration (FAA) has a requirement for the acquisition of the PAR-56 flashtube lamp NSN 6240-01-006-1260 used for runway approach lighting systems including: - Runway End Identifier Lighting System (REILS): Type FA-10264, Type FA-9628, Type FA-10096, Type FA-10229, Type FA-9437. - Medium Intensity Approach Lighting system with Runway Alignment Indicator Light (MALSR): Type FA-10290, Type FA-10097, FA-10267, Type FA-8981, and Type FA-11500. - Sequenced Flashing Light System: ALSF-2/SSALR: Type FA-9985, Type FA-10048, Type FA-10700. All work shall be accomplished in accordance to the attached Scope of Work. All assets will be shipped to the FAA Logistics Center (FAALC), Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK 73169. This requirement is being set-aside for small business. FAA contemplates award of an Indefinite Delivery/Indefinite Quantity type contract with a 1-year base period with four 1-year option periods. The NAICS code applicable to this requirement is 335110 Electric Lamp Bulb and Part Manufacturing. The business size standard for this NAICS is 1,250. Your attention is directed to Sections L and M of SIR/RFO 6973GH-19-R-00095 for instructions for preparation and submission of proposals. The SIR includes an evaluation of past performance. Offerors are required to provide Past Performance Surveys to customers/clients who are familiar with the work demonstrated through previous or ongoing contracts of a similar nature. The Past Performance Surveys will be returned directly to the Contracting Specialist. Offerors will each submit three (3) to five (5) Past Performance Surveys. [Provision L.2] Offerors will complete Pages 1 and 2 of the Past Performance Surveys and then provide the surveys to their customers, who will in turn complete and return the surveys directly to the Contracting Specialist by the due date specified on the surveys. Offerors shall give customers/clients sufficient time, recommended seven (7) days, to respond to surveys and submit to Contracting Officer prior to due date. Surveys received after the established due date may not be evaluated. Note: It is the responsibility of the Offeror to follow-up to make certain the surveys have been returned to the Contracting Specialist by the due date.
