1. Read box 28 and sign page 1 boxes 30 a, b, and c.

2. Provide your cage code.

3. Please quote FOB Destination Bremerton, WA.

4. Annotate the lead time.

5. Annotate the name of the manufacturer, if they are large or small business, and the products country of manufacturing.

6. Return this information with your quote.

 **If not currently registered in SAM for the following NAICS code,  314999 please register. 

 SOLICITATION MUST BE COMPLETED IN IT'S ENTIRETY AND RETURNED IN ORDER TO BE CONSIDERED RESPONSIVE.

IN ACCORDANCE WITH THE FOLLOWING ORDERING DATA;
"The vendor must provide a COC stating that either;
(1) The product was manufactured per MIL-C-24576, or;
(2) the product was tested per ASTM-D6413 and met the following requirements:
Maximum char length of 1.5 inches
Maximum after flame of 1 second
Maximum afterglow of 2 seconds
Must not exhibit any molten or flaming droplets"
