This is a request for non-personal service contract on a government purchase order for services of a Rheumatologist to see Native American patients on the Warm Springs Indian Reservation, Warm Springs Oregon.
STATEMENT OF WORK:
Contractor will provide a medical doctor, whose specialty is Arthritis, to conduct four clinics over the next five years at WSH&WC. They will provide 1-2 fellows to assist with these clinics. The Medical Doctor will provide a lunch time in-service for all providers. The fellows will provide an educational program for patients to better manage their lifestyle while living with Arthritis and will be available for consultation with the service unit medical doctor's between clinics to help with the medical managements of the patients.

The contractor shall provide professional liability (malpractice) coverage, in amounts that are reasonable and customary in the community for the appropriate specialty (not less than $1, 000, 000 per occurrence), covering liability for personal injury or property damage, including legal representation and expense of defense of any such liability claims, actions or litigation, resulting from participation by the Contractor's faculty and students under this agreement. This coverage may come from any source, but shall clearly cover the Contractor's faculty and students while participating under this agreement at the Warm Springs Health & Wellness Center.

The contractor and faculty must provide documentary proof of the insurance coverage to the Warm Springs Health & Wellness Center. The contractor and its faculty further agree not to seek indemnification from the United States or the Indian Health Service for any settlement, verdict or judgment results from any claim or lawsuit arising out of the performance of its faculty's professional duties while at the WSH&WC.
