This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The Government intends to acquire a commercial item using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13.

The Centers for Disease Control and Prevention contemplates award to a small business for renewal of What's Up Gold Service Agreements. The part numbers, license numbers, and ending dates of the current agreement are part of an attachment to this announcement. Please use this attachment in making your bid. Bids must only be for a 12-month period. You must bid the yearly fee against each line item in the order that it is on the attachment and then include a total for the entire procurement after each separate line item has been bid for the bid. This procurement is a total small business set-aside.

The NAICS Code for this procurement is 511210. The offeror shall state in their offer their size status for this procurement. Vendors shall be registered at SAM.gov and their Representations and Certifications shall be complete. If you have not completed Representations and Certifications at SAM.gov, please fill out the attached Representations and Certifications form attached. All responsible sources may submit an offer which shall be considered by the agency.
