perform this requirement in order to support a competitive procurement.
The Reynolds Army Community Hospital, Fort Sill, OK has a requirement for the following CPRR
Reagent/Service Agreement. Request for identifying potential small or large businesses capable
of delivering Routine, CPRR Reagent/Service. Vendor must properly handle, store, and transport
medical Reagent and provide service in accordance with all local state and federal requirements.
The applicable North American Industrial Classification System (NAICS) code is 334516,
Analytical Laboratory Instrument Manufacturing. The Small Business size standard is 1000.
Supplies/ Service: CPRR Reagent/Service Agreement Base Plus 4 option years Quantity: One (1) Job
Base 6 month: 1 May 2020 through 30 September 2020
Option 1: 1 October 2020 through 30 September 2021
Option 2: 1 October 2021 through 30 September 2022
Option 3: 1 October 2022 through 30 September 2023
Option 4: 1 October 2023 through 30 September 2024
The Government does not intend to make an award on the basis of this Sources Sought notice or
otherwise pay for the information solicited herein. This notice is for the purpose of identifying potential
sources and does not commit the government to make an award and/or issue a solicitation. Interested
and qualified firms shall submit the following information: company name, address, telephone number,
point of contact and CAGE/DUNS number for your firm. Include your socioeconomic category for
purposes of determining an acquisition strategy.

In response to this sources sought, please provide:

2. Identify whether your firm is interested in competing for this requirement as a prime contractor
or not. Identify subcontracting, joint ventures or teaming arrangement that will be pursued, if
any.
3. Information in sufficient detail regarding previous experience (indicate whether as a prime
contractor or subcontractor) on similar requirements (include size, scope, complexity,
timeframe, government or commercial), pertinent certifications, etc., that will facilitate making
a capability determination.
b. Identify any condition or action that may be having the effect of unnecessarily restricting
