Issuing Procurement Unit State of Utah Division of Purchasing Conducting Procurement Unit State of Utah Division of Purchasing REQUEST FOR STATEMENT OF QUALIFICATIONS Rangeland Services Request for Statement of Qualifications SOLICITATION #AS20026 This Request for Statement of Qualifications (âRFSQâ) is issued in accordance with State of Utah Procurement Code, Utah Code Annotated (UCA) Chapter 63G-6a, and applicable Rules found in the Utah Administrative Code (UAC). If any provision of this RFSQ conflicts with the UCA or UAC, the UCA or UAC will take precedence. Purpose of this Solicitation The State of Utah, Division of Purchasing has issued this RFSQ to obtain statements of qualifications from companies interested in providing various Rangeland Services, which include Mastication, Lop & Scatter, Chaining, Harrowing, Cut & Pile, Cut & Chip, Tamarisk/Russian Olive Removal, and Drill Seeding, with the intent to establish an open-ended approved vendor list. Vendors approved during this process will be prequalified for a period of 18 months after which time an approved vendor must resubmit qualifications for evaluation and renewal. This pre-qual list will open for new vendor submissions every twelve (12) months. The RFSQ can be reopened for any reason the State deems appropriate. State agencies requiring one of the Rangeland services above can, at that agencyâs option, refer to the pre-qualified list and limit any competition for any particular job to those on the pre-qualified list. Any State agency, at its sole option, may solicit bids from all interested vendors in addition to pre-qualified vendors, when it is determined to be in the Stateâs best interest to do so. This open bid policy will help provide opportunities for other vendors or new vendors to gain the experience necessary to be pre-qualified at a later date. Contractors who have completed projects for the State of Utah and received a performance review must have an average of 60 points in order to respond to this RFSQ. Contractors who are on the Current Rangeland Pre-Qual list (attached) and would like to pre-qual for other treatments and/or acreage are also invited to respond to this solicitation. Background It is the intent of the State of Utah to place qualified contractors on a pre-qualified list to be used for obtaining quotes or bids on various rangeland projects. These projects include Mastication, Lop & Scatter, Chaining, Harrowing, Cut & Pile, Cut & Chip, Tamarisk/Russian Olive Removal, and Drill Seeding. Issuing Procurement Unit, Conducting Procurement Unit, and Solicitation Number The State of Utah Division of Purchasing is the issuing procurement unit and conducting procurement unit for this RFSQ (referred to as âthe Stateâ). The reference number for this RFSQ is Solicitation #AS20026. This solicitation number must be referred to on all bids, correspondence, and documentation submitted to the State relating to this RFSQ. Detailed Work Requirements It is the intent of the State of Utah to place qualified contractors on a pre-qualified list to be used for obtaining quotes or bids on various rangeland projects. These projects include Mastication, Lop & Scatter, Chaining, Harrowing, Cut & Pile, Cut & Chip, Tamarisk/Russian Olive Removal, and Drill Seeding. In response to this solicitation, contractors can specify which type of project or projects he/she would like to be considered for. This could include one or more of the various types listed on Attachment F â Rangeland Job Categories and Sizes. Contractors placed on the pre-qualified list for each service must be capable of performing that project and must have all the skills, abilities, qualifications and equipment associated with that particular service or services. Performance evaluations will be completed for each project. An average performance evaluation score of less than 60 will be grounds for removal from the pre-qualification list for that specific treatment/acreage category and all larger acreage within that same treatment category. It is expected that all employees for any contractors placed on this list shall be qualified at all times to perform these services, shall work in a professional, safe and timely manner, and shall conduct themselves in a way that will promote a good working relationship with state employees and contractors. The Division of Purchasing will reserve the right to remove any contractor from the pre-qualified list at any time for failing to maintain these standards originally submitted with this solicitation process, or for any other reason deemed appropriate by the State. A contractor will have a job foreman/superintendent on the job site for all projects at all times. This individual must be able to speak English and shall have the authority to make decisions on behalf of the contractor. If required by the State, this individual must be bonded. The general job descriptions for each type of project can be found on Attachment B â General Job Descriptions. Additional instruction/information is provided on these attachments. A detailed scope of work will be provided with each project solicitation. Performance Reviews In accordance with Procurement Code 63G-6a-410(5)(e), the State of Utah has determined that it is in its best interest to conduct a contractor performance review for each rangeland project. The State agency project manager will complete a performance review according to the evaluation criteria in Attachment D. Vendors will be aware of their performance status as they will be monitored throughout the project. Any corrective action will be communicated to the vendor verbally or through a 10-day letter to cure, depending on the issues. Vendors must complete any corrective action requested or required if not, this will be reflected in their performance review scores. In order for a vendor to remain on the Rangeland Pre-Qualification list, they must maintain a minimum average performance score of 60 after 2 projects within the treatment category. If a vendor's average performance score drops below 60 within that treatment category, they will be removed from the pre-qualification list for that specific treatment/acreage category and all larger acreage within that same treatment category. If a vendor is removed from the pre-qualification list they may still be eligible to submit a bid on Open Bid solicitations. Once a vendor is removed from the pre-qualification list, the vendor may reapply at the next open RFSQ. Evaluation of Statements of Qualifications Each Statement of Qualifications received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. Pursuant to Section 63G-6a-407 of the Utah Procurement Code this RFSQ shall be evaluated by the objective criteria described in this RFSQ. Statements of Qualifications submitted must comply with the prerequisites and questions sections of this RFSQ. Any Statement of Qualification that does not comply with the prerequisite and questions sections will be deemed non-responsive and will be rejected. Any exceptions to the content of this RFSQ, including the prerequisites, must be protested in writing to the Division of Purchasing prior to the closing date. To be responsive and responsible Vendors must review and respond to the following sections of this RFSQ: Prerequisites, Buyer Attachments, and Questions. The Prerequisites Section contains the objective criteria that will be used to evaluate the bids, including the mandatory minimum requirements and general requirements that Vendors must certify that they have read, understand, and agree in order to submit a bid. The Buyer Attachments Section contains the required State of Utah Terms and Conditions of the RFSQ, which are non-negotiable, the General Provisions which apply to this RFSQ, and any additional required forms. The Questions Section contains the questions that Vendors are required to answer in order to submit their Statement of Qualifications. Vendors must review each section carefully.
