This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number N68095-15-T-3002, is therefore issued as a Request for Quote(s) (RFQ). This solicitation document incorporates provisions and clauses IAW the Federal Acquisition Regulation Circular 2005-74 and DPN 20140528.FAR Clause 52.211-6, Brand Name or Equal applies to this acquisition.

This proposed contract action is for one base year and one option year of Radiopharmaceutical Supplies for use in the Nuclear Medicine Department at Naval Hospital Bremerton, One Boone Road, Bremerton WA 98312-1898. The anticipated period of performance is 01October2015 - 30September2017. The associated North American Industrial Classification System (NAICS) code is, 325412, with a size standard of 750 employees.

This procurement will be solicited as a Total Small Business set aside, all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-53. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at http://farsite.hill.af.mil.
The brand name description of the radiopharmaceutical products is, Western Isotopes LLC. A description of each item is listed on the attached solicitation and Statement of Work.

The final unit prices established, resulting from this solicitation, when ordering as needed, will be applicable to any order placed under this contract. The Government will only pay for actual quantities ordered and accepted, regardless of the quantities established on the contract. Therefore, the offeror is hereby informed that the final unit prices are not subject to adjustment on the basis of a difference between ordered amount and actual quantities. Items will be ordered as necessary therefore quantities may vary. Please see Statement of Work attached.
