THE SOLICITATION IS AMENDED TO INCLUDE DOL WAGE DETERMINATION 2005-2431, REV. 5, DATED 09/2/8/2007, LABORER/GROUNDS MAINTENANCE, $9.07/HR PLUS HEALTH/WELFARE $3.16/HR. 100% Small Business Buy Indian Set Aside. Clinton USPHS Indian Health Center, Clinton, Oklahoma, has a one-year requirement for a laborer. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is a Request for Quotations and is numbered RFQ 246-08-Q-0014 which incorporates provisions and clauses in effect through FAC 2005-24. 100% Small Business, Buy-Indian Set Aside, NAICS code 561320. Line item 1 description: laborer services; estimated quantity 2150; unit of measure: hour. Description: Laborer services shall be provided at the Clinton USPHS Indian Health Center Facilities Management Department, Clinton, Oklahoma. The position is Monday through Friday with working hours 8:00 am to 5:00 pm with one hour for lunch. Services/laborer will be monitored by the Facilities Manager. Duties include grounds maintenance, maintenance repair helper, painting, drywall repair, basic electrical work, lifting, moving furniture/equipment, cleaning, sweeping and other duties assigned. Qualfications: laborer must be familiar/experienced with basic maintenance hand tools and hand and power landscaping equipment. Familiarity with Indian Health Service policies and procedures helpful but not essential. Evaluation: 1) price shall take precedence over technical qualifications; 2) experience based on submission of resume and checks; 3) submission of quote documents: proposed hourly price, updated resume with EIN and DUNS numbers, 2 reference letters, Buy Indian certification, copy of completed FAR 52.212-3; 4) CCR verfication.
