The Health Resources and Services Administration (HRSA) will be soliciting proposals to conduct a Demonstration Project to test the feasibility of establishing a structure for transplant recipients to receive follow-up care at HRSA funded health centers. This Demonstration Project requires a partnership between a transplant center and HRSA funded health centers. The transplant center will provide clinical education and provider support including a mechanism for the collection of follow up data on the transplant recipients.
This information will be collected from transplant recipients over 18 years of age, over a year removed from transplant, and have been free of a rejection episode, or any serious complications for 6 months prior. The information that will be exchanged between the transplant programs and the health centers will be patient confidential information that is subject to the requirements of HIPAA regulations. The period of performance will be a one year base with two (1) year options. This acquisition is being conducted under Full and Open Competition. The solicitation will be issued on or about 05 February 2007.
