(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-16-Q-0732 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. (iv) This acquisition is a total Small Business set aside. The NAICS code is 541611 and the size standard is $15 million. (v) The contract line items are listed in the price schedule of the solicitation package. When providing a quote, please provide a fixed rate for each line item. (vi) The contractor shall provide a consultant who will assist the VA Palo Alto Health Care System in their CARF accreditation in accordance with the attached Performance Work Statement. (vii) The contract will be for a base year plus two option years. The period of performance is listed in the Price Schedule and the Performance Work Statement. (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, does not apply to this acquisition. Evaluation will be based on the lowest priced technically acceptable quote.
