SOURCES SOUGHT ANNOUNCEMENT: The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 10, Milwaukee, WI 53214, is conducting market research to identify potential sources for Inpatient and Outpatient Therapeutic Apheresis Services at the Edward Hines Jr. VA Hospital, Hines, IL and the Jesse Brown VA Medical Center, 820 South Damen Avenue, Chicago, IL. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. As stipulated in FAR 15.201, responses to this announcement are not considered offers and cannot be accepted by the Government to form a binding contract. No solicitation exists; therefore do not request a copy of the solicitation. The decision to solicit for a contract shall be solely be at the discretion of the Government. The purpose of this market research is to identify qualified (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms; (4) Large business; (5) Non-Profit firms capable of providing Therapeutic Apheresis Services as described in the attached Performance Work Statement (PWS): All interested firms who can meet the requirements of the PWS should respond, in writing, with the following information: 1) Description of firm's capabilities and experience providing on-site therapeutic apheresis services. 2) Billing methodology for therapeutic apheresis services (CMS CPT Codes, all-inclusive per/procedure rate, etc.) 3) Acceptable Government Payment Methodology (Government Purchase Card, Purchase Orders, etc.) 4) Business category (SDVOSB, VOSB, Small Business, Large Business or Non-Profit) for which the firm is categorized Responses to the RFI shall be submitted to the Contracting Officer, Erica Powers at [email protected] no later than 12:30PM Central Standard Time (CST), 30 September 2014. Therapeutic Apheresis Services for the Hines VAH and Jesse Brown VAMC should be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRES WILL NOT BE ACCEPTED. The North American Classification System (NACIS) Code associated with this announcement is 621492 with a Small Business Size Standard of $35.5.M. Interested parties are reminded that in accordance with FAR 4.12, prospective Contractors shall complete electronic annual representation and certification in conjunction with FAR 4.11, requiring registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
