The Albuquerque Area Indian Health Service has a requirement for on-site optometry services at the Acoma-Canoncito-Laguna Service Unit optometry services at the Acoma-Canoncito-Laguna Service Unit, Acoma Pueblo Indian Reservation, San Fidel, New Mexico. Contractor to provide professional primary optometric care, including but not be limited to general eye examinations, vision screenings, specialty care, treating and monitoring ocular diseases. Services to be provided to the IHS beneficiares including children, adolescents and adults. Services will be provided by the Contractor under a personal services type contract for a base year with four, one-year options for renewal. Option renewals contingent upon evaluation of satisfactory services provided by the Contractor. Contractor must be a licensed Professional Optometrist in any one State of the United States, DC or a territory of the U.S. The Government anticipates in awarding a single contract. All proposals will be evaluated and considered for award. Technical proposals will be evaluated individually based on the nurmerically weighted technical factors. Technical proposal to be paramount over cost/price proposals. In the case of a technical proposal tie, fair and reasonable cost factors will determine the award of a contract.
