Request for Information (RFI)/ Sources Sought inquiry into your organizations interest in providing a Nephrologist to provide dialysis coverage for Veterans living in and around Grand Junction Colorado. The VISN 19 Rocky Mountain Network Contracting Office contemplates an award of a Requirements contract (firm fixed price) to provide services for VA beneficiaries living in the Grand Junction, CO area. The services will include dialysis coverage for the hospital 24 hours a day, 7 days per week to be provided by telephone and/or to see in-patients who are experiencing acute renal insufficiency or worsening renal insufficiency from a chronic condition. Services shall include clinical assessment including physical examination, review of all tests, and events, and consultations with colleagues involved in the case to ascertain a full clinical picture. The service may also require the institution of hemodialysis in such patients whereby the Nephrologist will review the plan of dialysis with the dialysis team and also review the results of dialysis including the patient's condition and biochemical markers. Dependent upon responses, the proposed contract award priority shall be to Service Disabled Veterans Owned Small Businesses, Veteran Owned Small Businesses and then Small Businesses. The period of performance for this contract will be one year, with additional four-one year option periods. The contract base year is estimated to begin February 2010.
