This notice is issued for the purpose of establishing a Blanket Purchase Agreement. The Department of Veterans Affairs, Network Contracting Office (NCO) 17 San Antonio, Texas intends to negotiate sole source with Polymedco, Inc. 510 Furnace Dock Road Cortland Manor, NY 10567, to perform and provide results for, but not limited to, the following procedures/tests: CPT Code: VA-CPT1, Test Description: Cost per Reportable ( 1 device per patient reportable) CPT Code: VAOC-CR, Test Description: Credit for Devices CPT Code: OCPU-VA1, Test Description: Personal Mailers 20 per box ( 1 device per mailer) CPT Code: OCS1, Test Description: Sampling Bottles ( 4 x 50 Sampling Bottles) CPT Code: OCS2, Test Description: Sampling Bottles ( 20 x 50 Sampling Bottles) CPT Code: TP-5, Test Description: Thermal Paper ( 5 Rolls per Box) in support of the Veterans Affairs Texas Valley Costal Bend HCS Pathology and Lab Medicine which includes VA McAllen Outpatient Clinic, VA Corpus Christi Outpatient Clinic and VA Ambulatory Surgical Center.. NAICS 621511 applies. The statutory authority permitting other than full and open competition is 41 U.S.C.253 (c) (1), as implemented by Federal Acquisition Regulation (FAR) subpart 6.302-1 (a) (2) only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized. The described will provide laboratory services to include analysis, reporting of analytic results, and consultation regarding selection, collection, transportation and result interpretation. No other company is expected to respond to this requirement. Period of Performance shall be from the date of award, and extend for three years. This notice of intent is not a request for competitive quotes. It is not a solicitation or request for offers. No solicitation package is available. However, any qualified vendor that determines it can meet the government's requirements may give written notification through e-mail to the Contract Specialist, [email protected] no later than September 27, 2013, 12pm CDT. No telephone responses will be accepted. Information received will be for the purpose of determining whether or not to conduct a competitive procurement. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer. A determination not to compete is solely within the discretion of the Government. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government.
