The Albuquerque Area Indian Health Service has a requirement for on-site clinic services for general and specialty ultrasound. The clinic setting is within the Acoma-Canoncito-Laguna Indian Hospital located on the Acoma Indian Reservation, San Fidel, New Mexico, Interstate 40, Exit 102. The services are required three days per week during the term of the contract. Each clinic day will be 8-hours long, or until the last patient has been seen. Services required include but is not limited to the listed procedures; Thyroid, Chest, Breast, Abdomen, Retroperitoneal, Obstetrics, Fetal Biophysical Profile, Transvaginal, Pelvic and Testicular ultrasound examinations. The contract will be awarded as a single, fixed-price requirements type contract for a base year with two, one-year options for renewal. The option years will be exercised contingent upon the existing need of the Government and the availability of funds. The Government shall provide the necessary equipment, to include but not be limited to HDI 3000 ATL Ultrasound machine, all ultrasound accessories, and supplies, telecommunications equipment as well as other hospital equipment necessary in the preformance of the contract. The contractor and his/her staff shall be licensed from the Reqistered Diagnostic Medical Sonographers (RMDS) and must be State licensed. The Request for Proposal (RFP) No. 05-01/VZ will be released on or about FEBRUARY 4, 2005 with a proposal due date as close of business, 4:30 MST on MARCH 3, 2005. The requirement is a TOTAL SMALL BUSINESS SET-ASIDE.
