Notice of intent to issue a five (5) year Indefinite Delivery Indefinite Quantity (IDIQ) contract. The Centers for Disease Control and Prevention (CDC) intends to conduct negotiations on a sole source basis with CHD Meridian Healthcare, 40 Burton Hills Blvd Ste 300, Nashville, TN 37215-6163. The purpose of this contract is to perform physical exams for NIOSH employees and maintenance of those records. This includes vital signs (height, weight, blood pressure, pulse, and respiration) as well as the actual physical exam and assessment performed by the physician and may include various vaccinations. This may include an assessment and review of systems (head, ears, eyes, heart, lungs, extremities, etc.), breast exam (if appropriate) and prostate exam (if appropriate). Other services provided include respirator clearance exams (may include spirometry and OSHA questionnaire review), SCBA clearance, hearing tests for mandatory hearing conservation program, testing for Beryllium exposure.
