The 460th Contracting Squadron, Buckley, AFB, CO 80011 is issuing this sources sought notice, in accordance with Federal Acquisition Regulation Part 10, for Dental Services to be provided within 6 statute miles for Buckley AFB. This sources sought notice neither constitutes a Request for Proposal or Invitation for Bid, nor does it restrict the Government to an ultimate acquisition approach. Also, this sources sought notice should not be construed as a commitment by the Government for any purpose. The contractor shall furnish a central facility or facilities, supervision, labor, materials, supplies, and equipment necessary to perform all dental services and associated support services for approximately 3,200 – 4,000 U.S. Service Members. Dental services are generally defined as preventive procedures, general dentistry procedures, and dental specialty procedures to include oral and maxillofacial surgery, endodontics, periodontics, and prosthodontics. All contracted dental practitioners, including Dentists and Dental Assistants, must possess a current, unrestricted, state of Colorado license to practice dentistry/dental hygiene. The North American Industry Classification System Code (NAICS) is 621210 with a small business size standard of $6 million. Potential offerors are responsible for monitoring this site for the possible release of the solicitation package. All interested parties shall submit their response, which shall not exceed 20 pages, by 15 February 2005 and include their: business name, brief company history, point-of-contact, DUNS and NAICS numbers, type of business (i.e. small, large, 8(a), ect.), and contract magnitude for any current or previous contracts. Also, all interested parties must be registered at the Central Contracting Registration (CCR) site, at http://www.ccr.gov, and must submit their representations and certifications at the Online Representations and Certifications Applications (ORCA) website at http://orca.bpn.gov.
