The Government desires to contract with a primary care provider in Harrisonburg, VA servicing the veterans in the targeted counties of Rockingham, Albemarle, Augusta and Greene Counties. Contractor's facility which provides services must be physically located within the city limits of Harrisonburg, VA. Contractor shall provide for the delivery and management of continuous primary and preventive medical and mental health care for enrolled VA patients. Contractor shall provide medical facilities, healthcare providers, medical equipment and supplies, limited pharmaceuticals and all the administrative functions in accordance with the terms and conditions of the resultant contract. Compensation to the contractor will be made through a quarterly capitation payment. All solicitation requests shall be submitted in writing to the Conracting Officer.
