(i) This is a combined synopsis/solicitation for commercial services prepared in accordance with Federal Acquisition Regulation (FAR) 12.6, and supplemental information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number is APPQWRFC-0004-5 and the solicitation is issued as a request for quotation (RFQ). (iv) This solicitation is set-asied 100% for small business concerns. The associated NAICS code is 541380 TESTING LABORATORIES, and the small business size standard is $10 million. (v) This requirement consists of 1 line item CARBARYL ANALYTICAL ANALYSIS: This contract is for the laboratory analysis of carbaryl in samples collected to determine the dermal and inhalation exposure of workers during loading or applications of carbaryl bait used for grasshopper suppression treatments. The unit of pricing is sample , the estimated quantity is a not to exceed amount of 300 samples. The pricing is Firm Fixed Price per sample up to the not to exceed amount. (vi) Work to be completed under this Purchase Order will consist of the following: Laboratories must analyze relevant sample matrices collected in conjunction with carbaryl bait applications, in accordance with the Federal Insecticide, Fungicide, Rodenticide Act (FIFRA) Good Laboratory Practice (GLP) Standards (40 CFR 160). Relevant matrices include AgHandlers Exposure Task Force (AHETF) provided inner dosimeters, gauze, OSHA Vertical Sampler tubes, and rinsate solutions, All matrices will be provided by APHIS or the AHETF. Laboratories must analyze samples in accordance with the analytical methods provide by APHIS and AHETF. (vii) Analysis of all samples should be completed in 28 days or less from date of awared. These methods will have been validated by the contract laboratory for use in the relevant matrices prior to the commencement of sample analysis Only APHIS and AHETF approved instrumentation, apparatus, and reagents may be substituted for those specified in the method. All substitutions must be clearly documented in the raw data. Qualifying laboratories must be able to handle at least 300 samples to be analyzed for carbaryl in a manner consistent with previous AHETF studies. (ix) The provision at 52.212-2, Evaluation Commercial Items (a) of FAR 52.212-2: Technical Capability, Past Performance and Price. Non Price related factors are significantly more important that price. In accordance with 52.212-1 (b) (4), provide a written narrative addressing the Technical Capabilities of the facilities proposed in the areas of this project. Include listings of professional certifications such as Good Laboratory Practice (GLP) Certifications. In accordance with 52.212-1(b)(10) provide references on previous projects of similar size and scale. Previous experience with other Federal Agencies is desirable. Provide Good Laboratory Practive (GLP) Certification in compliance with the US EPA FIFRA Good Laboratory Practive (GLP) Standards. THE CONTRACTOR SHALL RETURN A COMPLETED COPY OF 52.212-3 WITH IT’S QUOTATION. (xiv) This is not a DPAS rated contract. (xv) No Numbered Notes apply. (xvi) QUOTATIONS ARE DUE APRIL 29, 2005 BY 4 P.M.
