This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (hubzone, 8(a), small, small disadvantage, or service disabled veteran owned small business) relative to NACIS 621498, All Other Outpatient Care Centers (size standard of $19 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the Fedbizopps website. Responses to this sources sought synopsis must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (2) Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged business, Service Disabled Veteran Owned small business, Veteran Owned small business, large business. (3) Does your company have experience with the Department of Veterans Affairs capitated CBOC Program? (4) What supplies/services does your company primarily provide? (5) Will your company be a joint venture with another company? (6) How long has your company existed? All interested offerors will have to respond to the solicitation announcement, in addition to responding to this sources sought announcement. The Southeast Louisiana Veterans Health Care System (SLVHCS) intends to procure Primary Care, Women's Care, Preventive Care, Mental Health Care, Pharmacy, Social Work, and dietary counseling, annual flu vaccine, pneumococcal vaccine, health screenings, chronic disease management follow ups, gynecological exams, and work-ups for scheduled admissions to include services with space and sufficient administrative support services in the form of a Community Based Outpatient Clinic (CBOC). Services provided will include: Primary and Women's Care provided by 1.0 Physician Clinical (PACT) support provided by 1.0 RN 1.0 LPN 1.0 Clerical Associate Mental Health Care provided by 1.0 LCSW 1.0 Psychiatrist (on site at least 2 days per week and available by phone or CVT the rest of the week) Pharmacy Services provided by 1.0 Clinical Pharmacy Specialist (on site at least 2 days per week and available by phone or CVT the rest of the week) Dietician services provided by 0.2 Registered Dietitian Housekeeping Security The CBOC is required to be located in Bogalusa, Louisiana. This CBOC shall be solely dedicated to providing services to Veteran beneficiaries of the Southeast Louisiana Veterans Health Care System. The Contractor shall provide the facility, clinical staff, administrative support, equipment, and labor necessary for a clinical environment. The NAICS code for this procurement is 621498, All Other Outpatient Care Centers with a small business size standard of $19 Million. The Government intends to award a contract for services for the Bogalusa, Louisiana CBOC. The resultant contract will be firm fixed price indefinite delivery/indefinite quantity with a one-year base period and four (4) one-year option periods.
