The United States Property & Fiscal Officer (USPFO) for Arizona is seeking information regarding capability and availability of potential small business contractors to perform medical/dental services at specified locations in Arizona, primarily at Papago Park Military Reservation (PPMR), Phoenix, AZ 85008. The contractor shall provide all management, labor, material, equipment, supplies and certifications necessary to provide dental and medical services for Periodic Health Assessments (PHAs) for the Arizona Army National Guard (AZARNG) In Accordance With (IAW) Army Regulation (AR) 40-501, Standards of Medical Fitness dated 4 August 2011. This contractor will provide services necessary to meet Department of the Army medical and dental readiness for deployment standards to ensure mission capability of the AZARNG.

The contractor shall provide customized comprehensive dental and medical readiness to include PHAs and dental restoration. These services will be completed using contractor provided dental and medical equipment. Upon completion of services, medical information will be documented in accordance with Department of the Army Policy and be updated in the approved Army National Guard (ARNG) automated system (MEDPROS Web Data Entry (MWDE) access/entry MODS E-Profile access, Electronic PHA Upload) by the contractor within 10 business days.
The USPFO for Arizona is seeking qualified Small Business (SB) Community Members [Small Business (SB), Small Disadvantaged Business (SDB), Section 8(a), Veteran Owned Small Business (VOSB), Service Disabled Veteran Owned Small Business (SDVOSB), Women Owned Small Business (WOSB), HUBZone Small Business, Small Business (SB) led teams, Small Business Partnerships and or Small Business Joint Ventures] that qualify under the North American Industry Classification System (NAICS) Codes:
621999 - All Other Miscellaneous Ambulatory Health Services
The Small Business Size Standard is $15,000,000.
621111 Office of Physicians (except Mental Health Specialists). The Small Business Size Standard is $11,000,000.
621498 - All Other Outpatient Care Centers. The Small Business Size Standard is $20,500.000.

This Request for Information and Sources Sought Notice is part of Market Research. Your responses are sought to identify potential sources that have the knowledge, skill, equipment, experience and past performance necessary to perform the aforementioned medical services.
