The Department of Veterans Affairs VISN 9 Contracting Office, 3400 Lebanon Pike, Murfreesboro, TN 37129 is soliciting on behalf of the VA Medical Center, Lexington, KY for Contractor to provide Allergy Clinics either at the VA Lexington, 1101 Veterans Dr., Lexington, KY or at Contractor’s facility, located in Lexington, KY, two one-half day clinics per week for fifty weeks in a year. Contractor shall also provide allergy testing services at Contractor’s facility, only. Services shall be performed in accordance with the Statement of Work included in the solicitation. The resulting contract will be for one (1) base year with two (2) one year options to renew. Solicitation will be issued o/a 9/13/04 with quotes due o/a 10/4/04.
