Provide acute and chronic hemodialysis services for the VA Medical Center, Augusta, Georgia, for the period June 1, 2003 through May 31, 2004, with four (4) one-year options to extend. Acute hemodialysis services to be provided at the Augusta, Georgia VA Medical Center, and chronic hemodiaylis services to be provided at the Contractor owned facility. Contract will be all inclusive of facility and professional services. In addition to scheduled times, acute dialysis services shall be provided on an on-call basis upon two (2) hours' notice to Contract, twenty-four (24) hours per day, seven (7) days per week. The Contractor shall provide all equipment necessary to rend acute dialysis services. Contractor staff must be duly licensed and qualified physicians, nurses and/or qualified technicians trained in extracorporeal therapy including acute and chronic dialysis treatment as applicable to the particular service prescribed and in accordance with relevant state licensure requirements.
