To develop a program of psychosocial support services for Choices for Care (CFC) and MFP participants currently in community-based settings, as well as those transitioning from Skilled Nursing Facilities (SNF’s) to community-based settings, that will enhance on-going self-care and address social isolation of both participants and their caregivers. This will be done through contracting with up to 2 entities experienced with the use of Elder Care Clinicians to provide services and supports. These entities will pilot services expansion to include education and training about the value of self-care, provide access to mental health supports for individuals and their caregivers, provide access to substance use treatment screening and programs, and reduce social isolation and loneliness of individuals and their caregivers.
