This RFQ contains one line item: 0001 Service, Non-Personal, Psychiatric, 1 Feb -30 Sep 00, appx 1 day per week IAW attached Statement of work. Quotations will be evaluated in accordance with attached criteria. Offers should contain a quotation for 256 hours of service, resume and references of candidate.
