Description:
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) Solicitation number 20804_Musician_NFloresJ is issued as a request for quotation (RFQ).
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69.
(iv) This acquisition is reserved exclusively for small business participation. The applicable NAICS code is 813110 (Religious Organizations) and the small business size standard for this NAICS code is $7.0 Million.

(vi) The chapel at Andersen AFB requires the services of one Protestant Contemporary Service Musician. Please refer to the attached Performance Work Statement for more specific details.

Option 2, 08 October 2015 - 07 October 2016
(viii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Please refer to the attached Performance Work Statement for more specific details.
(ix) The Lowest Price Technically Acceptable (LPTA) source selection process will be used. Please refer to the attached Performance Work Statement for more details.
(x) Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer or be actively registered in SAM. Offeror shall submit information that includes, at a minimum documentation of the requirements of the PWS, specifically Section II "Qualifications". Offeror must also participate in a interview via teleconference or in person.
(xi) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
