The service provider will support four individuals with moderate to severe cognitive impairments who have severe behavior challenges and some health needs. The individuals will be transitioning from Porterville Developmental Center – Secure Treatment Program (PDC-STP) or in some circumstances may serve individuals who are in jail and are being diverted from PDC. RFP for CPP Project 2021-2: Project 2021-2: San Diego Regional Center (SDRC) is seeking a proposal for a Housing Development Organization (HDO) entity to purchase an existing single family home in San Diego County, and retrofit the home with any adaptations needed for the individuals who are targeted to live in the home. SDRC seeks to develop a home that will be used as a Enhanced Behavioral Support Home (EBSH) and house 4 regional center clients. The home that is purchased and renovated by the HDO will be leased to a service provider who will provide the needed supports to the individuals who live in the home. The service provider will be selected through a separate Request for Proposal process. Project 1920-11: San Diego Regional Center (SDRC) seeks a service provider to develop a Community Crisis Group Home to support four children in need of crisis intervention services. The service provider will support four individuals with moderate to severe cognitive impairments who have severe behavior challenges and some health needs. The individuals are presently living in a developmental center or at risk of entering one. The funds available are up to $150,000, which will be deferred to FY 18/19. Project #6: This specialized residential facility will be owned by a non-profit housing development corporation (NPO). The service provider will support four adolescents with autism and severe-mild cognitive impairments who cannot be supported in typical level 4 facilities due to their challenging behaviors.
