1. - Services to be Provided, Details: 1. Location of Service: Different locales throughout the Commonwealth of Kentucky and states of Tennessee, Ohio, and Indiana. 2. Period of Service: January 23, 2023, to June 30, 2023. This contract may be extended at the completion of the initial contract period for 1 additional one-year period. 2.1. Vendor must provide a copy of credentials to serve individuals in the Commonwealth of Kentucky and states of Tennessee, Ohio, and Indiana. 3. Services provided by Vendor/Contractor/Proprietor: The service provider will provide the following services: 3.1. Process of service to individuals designated by the Office of Kentucky Law Enforcement Council Support (OKLECS). 3.1.1. Process of service includes three (3) attempts to serve individuals. 3.2. Process of service to individuals designated by the Kentucky Department of Criminal Justice Training (DOCJT). 3.2.1. Process of service includes three (3) attempts to serve individuals. 3.3. Provide last minute service requests within five (5) business days from the process server's receipt of request. 3.4. Provide change of address information for individuals. 3.5. An Affidavit of Service must be provided to requesting office (OKLECS or DOCJT) after service is complete. 4. Items/Supplies/Equipment provided by Provider: The service Provider will provide all necessary supplies. 5. Inspection and Acceptance of Services: The agency representative will approve the provider's invoice before payment is authorized.
