Norfolk Naval Shipyard (NNSY) has a requirement for Prescription Safety Glasses to employees as prescribed. Each order consists of frame, lenses, and a soft case with clip. Eye technician/clerks at the NNSY Naval Medical Clinic will complete the vendor's order form. The order form will be electronically transmitted via email to the vendor. Once the order has been completed, the vendor will ship the order with attached copy of the order form, within 5-7 working days to NNSY Naval Medical Clinic located in Portsmouth VA 23709-5000. See attached statement of work an eyeglass estimate for one full year and four (4) option years to renew contract and SF1449 Solicitation. Period of Performance for base year will be 3/20/19 - 3/19/20, Option year one (1) is 3/20/20 - 3/19/21, Option year two (2) is 3/20/21 -3/19/22, Option year three (3) is 3/20/22 -3/19/23 and Option year four (4) is 3/20/23 -3/19/24. Quotes should include applicable shipping charges vendor tax I.D, Cage Code and DUNS number. Pricing should reflect the full year and the option years to renew. This requirment will be restricted to the Federal Prison Industries (FPI) IAW FAR Part 8.
