The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Podiatrist to provide services at the White Earth PHS, IHS Health Center in White Earth, Minnesota. Duty hours are eight (8) hours per work day from 8:00am to 4:30pm to be scheduled Monday through Friday. The Contractor shall work approximately two (2) days per week as needed. The period of performance will be Date of Award through September 30, 2009 or 350 hours; whichever comes first. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. The Request for Quotations (RFQ) package will not be posted on FedBizOpps. Requests for the RFQ package, RFQ-09-041, shall be in writing either by email to [email protected] or by fax to 605-226-7669. In the written request please reference RFQ number RFQ-09-041 and provide a postal mailing address, email address, and fax number where the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be February 3, 2009. The closing date of the RFQ package is February 18, 2009.
