This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, and as supplemented with additional information included in this notice. Vendors must bid on all items listed in the attachment. The Government reserves the right to make award on an "all or none" basis. Vendors must quote Brand Name or Equal. Equal items may be modified to meet or exceed brand name items. Any quote that contains anything other than the brand model numbers described in the attachment must provide all specifications for technical evaluation. Items must be brand new; no remanufactured or refurbished items accepted. Bid/offer MUST be good for 60 calendar days after submission. Agencies must be registered with the Central Contractor Registry (CCR) (www.ccr.gov).

Offerors are reminded to complete and return with their quotes, all required certifications contained in this solicitation. All Offerors wishing to do business with the Government must possess a valid DUNS number and Cage Code and must be registered in the Central Contractor Registration (CCR).

Prices must include shipping/delivery costs to Bagram Airfield, Afghanistan and Kandahar Airfield, Afghanistan. An Amendment has been posted to reflect corrections to the delivery to the following locations and quantities:
• Bagram Airfield, Afghanistan - 4 each Plasma Freezers; 2 each Blood Bank Refrigerators.
• Kandahar Airfield, Afghanistan - 2 each Plasma Freezers; 2 each Blood Bank Refrigerators.

Prior to submitting offers, Offerors must ensure they review and acknowledge the terms, provisions, and conditions in the accompanying attachment, SF 1449 Solicitation W91B4N-10-T-5043 and any follow-on amendments.
