The 56thContracting Squadron Luke AFB intends to issue under FAR Part 12 a sole source purchase order to Oculus, Inc., 17721 59thAve NE, Arlington, WA 98223-6446, to provide Pentacam® HR Premium which will be used to for pre-operation screen for patients at 56th Medical Group, Optometry Clinic. The Pentacam® HR Premium supplies topographic data on the elevation and curvature of the entire anterior and posterior corneal surface, which is needed to properly refer patients for surgery.
The proposed contract action is for medical supplies for which the government intends to award to one source under authority of FAR 13.106-1(b) - Soliciting From a Single Source. Interested persons may identify their interest and capability to respond to this requirement. Responsible sources may submit a written narrative statement of equivalent system capable of providing topographic data of both the anterior and posterior corneal surface. To be considered an affirmative response, offerors must provide sufficient information to permit agency analysis to establish a bona fide capability to meet the requirements. Responses without the sufficient documentation noted above will be considered non-responsive. Telephonic or faxed responses will not be accepted.
