Contractor will create a customized Criteria File using their database based on a set of criteria (primarily industry and employment-based) defined by OSHA.

Award would be based on (Lowest Price Technically Acceptable) LPTA

To verify the data is technically acceptable, the U.S. Department of Labor 3 recent past performances and a sample file for review. Please provide a sample file with all of the data elements required by the SOW for the following SICs in the indicated states:
SICs: 3272, 2011(one or more employees) and 5013 (10 or more employees)
States: CO, PA, and DE

Please provide the file in Excel, csv or txt delimited format. If the file cannot be delivered in one of these formats, please indicate a format prior to delivery.
