The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for an Orthopedic Surgeon to provide services at the Pine Ridge PHS-IHS Hospital in Pine Ridge, South Dakota. The period of performance will be from Date of Award through September 30, 2010. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-081, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-10-081 and provide a postal mailing address or email address and state preference as to how the RFQ package should be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be December 23, 2009. The closing date of the RFQ package is January 7, 2010.
